Summary

The Social Work Supervisor is responsible for the administrative and clinical oversight of assigned social workers within primary care and related ambulatory care setting. The incumbent provides high level leadership in the planning, coordination, implementation and evaluation of social worker services to ensure delivery of high quality Veteran centered care.

Duties

Total Rewards of a Allied Health Professional Duties include, but are not limited to:

  • Clinical Functions (20%): Independently completing comprehensive psychosocial assessments to evaluate Veteran’s functioning, support systems, biopsychosocial stressors, and barriers to care.
  • Providing individual or group interventions to address adjustment to illness, mental health or substance use concerns, grief and loss, advanced care planning, housing instability, caregiver stress, and other psychosocial needs.
  • Delivering crisis intervention, risk assessment, safety planning, and coordination with mental health or emergency services as indicated.
  • Coordinating case management services, inclusive of care planning, facilitating access to resources and working collaboratively with PACT and interdisciplinary team members.
  • Administrative and Supervisory Functions (80%): Establishing clear performance standards and expectations though the performance management system.
  • Observing employee performance, providing ongoing feedback and complete formal competency evaluations and performance reviews.
  • Conduct clinical supervision and consultation to guide staff in managing complex psychosocial cases and implementing evidence based social work practice.
  • Assigning, coordinating, and monitoring workload to ensure equitable distribution, efficiency, and quality care delivery.
  • Supporting staff in the adoption of evidence based practices, Veteran centered care principles, and PACT aligned approaches to population health and chronic disease management.
  • Reviewing and approving/disapproving leave an ensuring adequate coverage across clinical areas.
  • Developing, implementing, and evaluating policies, procedures, workflows, and guidelines to enhance service efficiency and quality.
  • Using data systems to track service delivery, workload capture, utilization patterns, productivity, and Veteran outcomes Work Schedule: Monday-Friday, 8:00am-4:30pm Recruitment Incentive (Sign-on Bonus): Not Authorized Pay: Competitive salary and regular salary increases.
  • When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
  • Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year).
  • Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
  • Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
  • Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
