Summary
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
Duties
- VA Careers – Social Work: https://youtube.com/embed/enRhz_ua_UU The incumbent will function as part of the interdisciplinary treatment team at the Dom VA Medical Center within the Home-Based Primary Care (HBPC) Program.
- The HBPC Social Worker provides and coordinates health care services to Veterans in a home environment, residential care setting or virtual video connect (VVC).
- The incumbent will coordinate linkage with appropriate VA and community service providers or agencies as needed by the client.
- This is accomplished in collaboration with other members of the interdisciplinary team as appropriate.
- The HBPC Social Worker provides coordination of care for Veterans at the Dom VA Medical Center who are in the HBPC Program.

Requirements
