About the Position: This position is with the Defense Health Agency, Defense Health Network Indo-Pacific, at Health Clinic, Camp Humphreys, Republic of Korea. This is a Direct Hire Solicitation. Duties
- Perform intake mental health screenings to gather patient social histories, assess immediate needs, and collect factual information for clinical providers.
- Maintain ongoing follow-up communication and supportive care with high-risk patients to implement prescribed treatment goals and monitor transitions from inpatient to outpatient care.
- Facilitate and coordinate psycho-educational and support group sessions to enable comprehensive group care for targeted client populations.
- Collaborate with multidisciplinary mental health teams, military command, and case managers to coordinate care and ensure maximum utilization of mental health resources.
- Process patient referrals by retrieving data from electronic systems and compiling patient charts to assist providers in determining the appropriate scope of care.
- Serve as a patient advocate by providing education on prescribed treatments and assisting patients in navigating complex military healthcare systems and community resources.
- Utilize digital health tracking systems to ensure comprehensive record-keeping and execution of follow-up plans.
- Monitor clinical data and track patient outcomes to identify trends or problems at the client/system level.
- Evaluate, document, and report issues related to continuous quality improvement and risk management to assist leadership in maintaining standards of care.
