Summary

The Community Care Registered Nurse (RN) functions directly to support the activities of the St. Cloud VA Health Care System Community Care. The RN is responsible for the planning, integrating, implementing, modifying and evaluating the effectiveness of the Veteran’s care. The Community Care foundational elements will serve to support delivery of high quality, timely care to the Veteran receiving services.

Duties

Major duties include, but are not limited to:

  • Assesses clinical appropriateness and appropriate level and access to care, reviews and process referrals timely within scope of practice.
  • Demonstrates effective communication skills and communicates plan of care effectively while monitoring transitions of Veteran safely within their scope of practice throughout the EOC.
  • Serves as the Veterans’ clinical main point of contact for community care and traveling Veterans requesting local care and services.
  • Contacts Veterans/families and provides Veteran/care giver education related to consults or follow-up activities.
  • Serves as liaison between referring provider, community provider, and between VA’s for traveling Veterans.
  • Collaborates with VAMC Integrated Community Clinic (ICC) Care Coordinators, Service Lines (SL), and community providers to ensure the Veteran receives appropriate care.
  • Facilitates handoff to community provider and/or internal providers/team to assist Veteran with needs (i.e. pharmacy, durable medical equipment).
  • Performs clinical review of consults using utilization management principles including evidence-based medical, Interqual criteria, pre-appointment requirements and/or local guidance.
  • Conducts a comprehensive case management assessment within specific Veteran populations and post appointment follow-up to identify future needs.
  • Utilizes comprehensive knowledge and demonstrates proficiency in all community care processes (forms, tools, documentation).
  • Demonstrates understanding of regulatory information, eligibility criteria, confirms Veteran enrollment and authorization of community care referrals according to VA policies and directives.
  • Directs and facilitates scheduling of VA and community care appointments.

Requirements

  • U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
