Summary
The primary purpose of the Licensed Practical/License Vocational Nurse (LPN/LVN) is to perform assigned duties in a manner that promotes quality health care, positive customer service, and the maintenance of an environment committed to serving the veteran.
Duties
- The LPN/LVN also performs assigned duties in a manner that promotes quality health care, outstanding customer service, and the maintenance of an environment committed to serving the veteran.
- The LPN/LVN performs within this functional statement under frequent guidance of the Nurse Manager or designee.
- Assigned tasks require basic technical competence and skill.
- The Licensed Practical Nurse (LPN/LVN) assigned PACTNICTOR teams at OPA 1 & OPA 2, Mayaguez Outpatient Clinics (MOPC), Ponce Outpatient Clinic (POPC), Arecibo CBOC (ACBOC), Ceiba CBOC, Guayama CBOC(GCBOC), including coverage of St. Croix CBOC (STX CBOC); and Rural clinics provide safe, individualized, and integrated holistic practical nursing care to patients who are acutely and/or chronically ill in the Outpatient care setting, such as renal, cardiological, neurological, pulmonary, GI, surgical, dermatological, orthopedic and endocrinologic conditions.
- Care is provided to patients who exhibit a variety of physical problems.
- In addition, ambulatory care patients who present behavioral problems will benefit from a Whole Health care delivery system in Primary Care, in which a partnership to reach wellbeing is developed with Primary Care teams, families, and other professionals working with Primary Care Mental Health Integration (PCMHI), and Suicide Prevention program.
- The LPN/LVN functions under the direct supervision of the Registered Nurse (RN) who is responsible for the coordination of the patient’s care.
- LPNs at the are expected to perform all duties within the scope of their license.
- They are expected to exercise greater judgment require less supervision and operate by all established policies and procedures.
- The LPN/LVN will implement appropriate nursing actions based on the plan of care and his/her observations of the patient’s condition.
- The LPN/LVN serves as a member of the health care team in the Outpatient care setting.
- The LPN/LVN recognizes the need for and seeks guidance when necessary to ensure quality patient care.
- In organizing and delivering care, the LPN/LVN recognizes and considers emotional, cultural, spiritual, and age-related factors.
- He/she will demonstrate increasing levels of responsibility as he/she actively progresses up the career ladder.
- The LPN collaborates with the patient and Sub-Specialty Providers, family/caregiver, internal and community-based services involved in providing care to the patient) in developing a patient-driven holistic care plan for life.
- Provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient’s goals.
- We also provide a full range of primary and preventive women’s health care, including Gynecological and Reproductive Care (except for OB and infertility services), evaluate patients’ reminders due, to determine needs of mammography, pelvic exams, breast exams, densitometry as well as sexual trauma counseling among other interventions.
- Work Schedule: Monday through Friday 7:30 am – 4:00pm Telework: Not Available.
