Nurse (Patient Aligned Care Team, Outpatient)

Summary

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

Duties

• Grants Pass is a city in and the county seat of Josephine County, Oregon, United States.
• The city is located on Interstate 5, northwest of Medford, along the Rogue River.
• The population is 39,194 according to the 2020 census, making it the 15th most populous city in Oregon.
• The lumber industry was the major employer for Grants Pass up until the early 1970s.
• At that point the entire region started to see a steady decline in all lumber harvesting, production, and processing.
• Since then there has been a shift to a large service industry sector covering areas of outdoors/sports/recreation and health care infrastructure.
• This is augmented by multiple small and medium businesses and growth in marijuana-related businesses due to state legalization.
• Functions or Scope of Assigned Duties include but are not limited to the following: Assesses, plans, implements, and evaluates care based on age specific components.
• Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
• Administers medications and procedures per established policies and guidelines.
• Influences care outcomes by collaborating with members of the interdisciplinary team.
• Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex primary care.
• Provides sex-specific direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department.
• Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels.
• Provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management.

Requirements

U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA.
