  • VA Careers – Pharmacy: https://youtube.com/embed/Fn_ickNBEws The primary responsibility of the Clinical Pharmacy Specialist is the provision of evidence-based pharmaceutical care services through both direct and indirect interaction with patients and providers.
  • This pharmacist promotes and manages drug therapy based on current clinical knowledge that is consistent with policies established at the national, VISN, and local levels.
  • A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify, or discontinue medication therapy and as a consultant for intensive medication therapy management services.
  • This includes, but is not limited to the following: Designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments.
  • Helping achieve positive patient centric outcomes through direct and indirect interactions with patient's providers, and interdisciplinary teams in assigned areas.
  • Ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy.
  • Exhibiting initiative and resourcefulness in managing assigned workload.
  • Providing high quality, patient centric care using evidence-based medicine to ensure safe, effective and efficient medication therapy.
  • Interacting professionally with all customers Incorporating the VHA mission, vision and goals into own priorities and practice.
  • Maintaining current, comprehensive knowledge of evidence-based practices, and actively pursues opportunities for continuous professional development.
  • Additionally, the CPS performs other duties as assigned.

Requirements
