Duties
- As the Home and Community-Based Care (H&CBC) Registered Nurse assigned to the Veteran Directed Care (VDC) program is responsible for providing care coordination and program monitoring for Veterans with high levels of functional, medical, and psychosocial needs.
- This role ensures Veterans receive safe, high-quality, and cost-effective care while maintaining autonomy and remaining in their preferred care setting.
- Duties of the VDC RN include, but are not limited to: Assessing and determining clinical appropriateness of Veterans for enrollment in the Veteran Directed Care program.
- Implementing and applying the Case Mix Index (CMI) to establish appropriate levels of care and budget allocations. Developing, reviewing, and monitoring individualized Veteran budgets (spending plans). Monitoring the quality and safety of care provided to Veterans enrolled in VDC.
- Conducting quarterly reviews and complete required documentation for Personal Care Services. Maintaining ongoing communication and coordination with the VDC Provider(s). Attending and actively participating in monthly VDC Provider meetings.

Requirements
- U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

Education
The Associate Degree Nursing (ADN), Bachelor of Science degree in Nursing (BSN), or Master of Science degree in Nursing (MSN) must be fully accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission On Collegiate Nursing Education (CCNE).
