Summary

The Primary Care Provider (Nurse Practitioner/NP) provides comprehensive clinical care, consultation, and patient management in support of the VA mission. They practice to the full extent of their training and certification, follow defined clinical privileges, and hold prescriptive authority for their assigned patient panel or program.

  • The Primary Care Provider (NP) is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients.
  • Duties of the position include are but not limited to: Manage a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice.
  • Develop and implement individualized plans to achieve optimal population health and disease management.
  • Demonstrate leadership and work collaboratively with Primary Care Team and other interdisciplinary team members.
  • Order and interpret labs, tests, and diagnostic studies.
  • Prescribes pharmacologic and non-pharmacologic interventions.
  • Initiate referrals and consultations to specialty care services.
  • Provide education to patients and their families.
  • Assess and adjust the plan of care as needed to achieve optimal patient outcomes.

  • Influence and model the delivery of evidence-based health care, demonstrate expertise in the ability to manage a patient panel, population, or program.
  • Initiate and lead interdisciplinary teams informed by structured approaches and processes to improve outcomes that positively impact care for a patient panel, population, or program.
  • Acquire knowledge and skills, then disseminate acquired information within and/or outside the medical facility.
  • Initiate and lead interdisciplinary teams developing systems that positively impact outcomes at the panel, population, or program.
  • Demonstrate effective communication skills and professional behavior, promoting cooperation and teamwork.
  • Meet the needs of customers while supporting the medical center and service missions.
  • Communicate and treat customers (patients, visitors, volunteers, and all medical center staff) in a courteous, tactful, and respectful manner.
  • Provide the customer with consistent information according to established policies and procedures.
  • Handle conflict and problems in dealing with the customer constructively and appropriately.

Requirements

  • U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
