Summary

The Community Housing and Outreach Services (CHOS) Social Work Supervisor ensures for program integrity, overseeing the utilization of Housing and Urban Development – Veteran Affairs Supported Housing (HUD-VASH) Program vouchers in their coverage area within the VA Puget Sound. May also supervise staff from other CHOS programs, as assigned.

Duties

  • The supervisory social worker is responsible for the professional and administrative management of an area in a social work service, in a care/product line, or multiple facilities.
  • They provide direct clinical services with or without program coordination responsibilities.
  • The Community Housing and Outreach Services (CHOS) Social Work Supervisor ensures for program integrity, overseeing the utilization of Housing and Urban Development – Veteran Affairs Supported Housing (HUD-VASH) Program vouchers in their coverage area within the VA Puget Sound. develops, implements, and documents the psychosocial assessment, treatment plan, progress notes and updates, and discharge planning for provision of clinical services to Veterans, including prompt completion of clinical reminders provides consultation to health care providers about assessment findings and treatment plans for Veterans, so that care is coordinated, collaborative, and integrated within the medical center. provides leadership in identifying and addressing ethical issues that impact patients and staff, including initiating and participating in ethical consultations The incumbent is responsible for furthering the goals of equal employment opportunity (EEO) by taking positive steps to assure the accomplishment of affirmative action objectives and by adhering to nondiscriminatory employment practices in regard to race, color, religion, sex, and affirmative action monitors compliance of staff members with VA performance measures and effectively shapes provider practice patterns to improve standing on performance measures.
  • Performs HUD-VASH voucher reconciliation and monitoring of utilization for geographic area (may include multiple public housing authorities (PHA)).
  • Monitor and manage electronic medical homeless consults for geographical area as assigned.
  • Incumbent participates in the hiring of new staff members for the team (writes announcements for posted positions, prepares functional statements, and may assume primary responsibility for selecting final job candidates). shares expertise with internal and/or external customers to the organization by leading or actively participating in interdisciplinary groups such as committees, taskforces, projects, formal teaching, publishing, or professional activities.
  • Incumbent works with staff to effectively shape provider practice patterns to improve standing on performance measures.
  • Consistently relates to supervisors, co-workers, patients, visitors, and the general public in a courteous and cooperative manner, contributing to the effective operation of the social work program.
  • Effectively communicates with and utilizes other disciplines (e.g., nursing, medicine) to facilitate treatment planning and implementation.
  • With few exceptions, participates effectively in team meetings, treatment planning conferences, etc. Work Schedule: Monday Thru Friday 8:00 am.-4:30 pm.

  • Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
  • Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.

Requirements
