Summary

This position is eligible for Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

Duties

  • The Hud-vash program, a partnership between the Department of Housing and Urban Development (HUD) and the Depart of Veterans Affairs, is a clinical case management program for chronically homeless Veterans aimed at providing psychosocial services to Veterans who acquire permanent housing through HUD's Section 8 voucher system.
  • The job functions and responsibilities are listed below but not limited to: Function as a Case Manager for a caseload of Veterans admitted into the Hud-vash program, working with a multi-disciplinary team to assist the Veteran to secure and maintain permanent housing; Determine the stage of case management for each Veteran and confirm that the multi-disciplinary team is meeting face to face with the Veteran at the frequency required by the given stage and understand that these face-to-face meetings should take place in Veteran's residence unless otherwise indicated; Ensure that the team's interventions adhere to the treatment plan created with the Veteran.
  • In addition, to complete the other documentation requirements of the Hud-vash program, including psychosocial assessments, HOMES database updates, measurement based care tracking, pain, nutrition, and suicide screenings; Demonstrate the ability to independently conduct a psychosocial assessment with a wide variety of individuals from various socio-economic, cultural, ethnic, educational, and other diversified backgrounds.
  • The assessment includes documentation of identified behaviors or symptoms of abuse, neglect, exploitation and/or intimate partner violence, as well as strengths, limitations, coping skills, and internal/external supports and service needs to optimize the Veteran's functional status; Develop the assessment in collaboration with the interdisciplinary treatment team, the Veteran, available family/friends/caregivers and collateral sources such as CPRS, HOMES, and agencies with appropriate ROl's ; Identify family/caregiver stressors and provide specific interventions.
  • Time frames and requirements for assessments are completed within time frames as outlined by program and Social Work Service policy, and accreditation agencies such as Joint Commission and CARF; Possesses knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic injuries/illnesses, common medications and side effects, and medical terminology) to formulate a treatment plan; Act as an advocate with appropriate VA and community service providers and agencies when it serves the best interest of the Veteran and family members/caregiver; Conduct timely assessment of at-risk Veterans in crisis to identify immediate needs, evaluate risk, and initiate safety plan as appropriate; Provide interventions independently with Veterans and their families/caregivers who are experiencing a wide range of complicated medical, behavioral health, financial, legal, and psychosocial problems; Provide specialized consultation to colleagues and students on the psychosocial treatment of patients in this service delivery area as well as role modeling effective social work practice skills; Provides leadership, direction, orientation, coaching, in-service training staff development and continuing education programs for social work staff Duty Location: 630 Kings Court, Ukiah, Ca 95482 Work Schedule: Full-Time, Monday-Friday 8am to 4:30pm or determine by the supervisor.
