  • This position is 70% patient care and 30% program coordination and administration.
  • The incumbent is responsible for leading and developing strategic outreach in response to available data related to homelessness in collaboration with community agencies and within the VA system.
  • The incumbent is responsible for managing local outreach calls, overseeing data for local response to National Call Center for Homeless Veterans.
  • The incumbent provides monitors the department's afterhours homeless outreach phone and provides on call services as assigned.
  • The incumbent is responsible for developing collaborations with community agencies serving the homeless, as well as conducting independent street outreach to engage Homeless Veterans.
  • Duties include, but are not limited to: Organizing and monitoring micro and mezzo Veteran outreach to homeless and at-risk Veterans within assigned areas; develops reoccurring and responsive outreach for assigned areas; leads strategic planning including real time safety data analysis and program response for homeless Veteran Outreach.
  • Leading the homeless Veteran outreach team in performance improvement and sustainment activities to optimize team efficiency and outreach services for Veterans; monitors service mapping and duplication of service through comparable programs such as SSVF.
  • Providing independent clinical outreach services at the full extent of the advanced practice clinical level, within privileges, credentials, scope of practice, elements of practice, certification, functional statement, position description, o other VHA or local facility approved documentation of competency.
  • Completes community diagnostic and assessments in nontraditional settings for those Veterans unable to access traditional in office clinics.
  • Develops communication platforms with team members and community agencies (including Veterans and caregivers) to convey clinically sensitive information for the care of the Veteran, ensuring compliance with VA privacy standards.
  • Develops community-based outreach activities to target tent encampments and other identified hotspots that require Homeless Veteran engagement appropriate to geographic locations; deploys research-based strategies for safe and effective rural and inner-city settings, data entry and reconciliation with HMIS and other community homeless data systems.
  • Maintains complex clinical caseload of Veterans identified as vulnerable and in need of advance practice community based clinical services.
  • Performs other duties as assigned.

Requirements

Benefits

Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade). Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
