Duties
- On the Nurse Practitioner NG 61 pay scale The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) leads facility efforts in population management for health promotion and disease-management initiatives.

- Major duties and responsibilities of the position: Clinical Care Duties Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice Develops and implements an individualized plan to achieve optimal population health and disease management.

- Orders and interprets labs, tests, and diagnostic studies.

- Leadership and Advocacy Leads efforts to identify key challenges and priority areas for improvement, using data and performance metrics to guide decision-making and advocate for effective interventions.
- Serves as a primary point of coordination for system-wide preventive-health initiatives and ensures that facility teams are informed, prepared, and engaged in implementation.
- Builds and maintains collaborative relationships with internal and external partners to support Veteran wellness and expand the reach of preventive services.
- Program Development and Implementation Oversees the design, implementation, and evaluation of health promotion and disease-management programs, ensuring they are evidence-based and responsive to local Veteran needs.
- Develops structured action plans, workflow processes, and standardized tools to support consistent delivery of preventive-care services across primary care settings.
- Monitors program performance using data dashboards, quality measures, and outcome reports, and adjusts strategies to ensure continuous improvement.
- Ensures effective communication and coordination across care teams so that preventive-health services, behavioral-health interventions, and wellness activities are implemented consistently throughout the facility and affiliated clinics.

Requirements
