Summary

This position is located at VA Central California Health Care System, Fresno, CA. The Licensed Vocational Nurse (LVN) practices in the outpatient care setting under the direction and supervision of the Clinical Nurse Manager.

Duties

  • Total Rewards of a Allied Health Professional the LVN is a member of the Patient Aligned Care team (PACT) consisting of (Veteran, Provider, RN Care Manager, Clinical Associate and Clerical Associate) and Expanded Team (Family/Caregiver, Social Worker, Pharmacist, Dietician, Internal and Community-Based Services) involved in providing care to the patient and in developing the PACT plan.
  • Major duties include: Demonstrates knowledge and ability to vary the order and sequence of assigned procedures based on patient's condition Uses judgment in selecting the most appropriate application of procedures and guidelines Reports and records significant patient information.
  • Observes, identifies and responds to the patient's needs for nursing, medication, and equipment-assisted care and to recognize deviations from normal.
  • Considers physical, emotional and social factors in modifying patient care plans.
  • Prepares, administers and documents actions taken specific to commonly prescribed oral, topical, intradermal, subcutaneous, and/or intramuscular mediations, immunizations or skin tests as permitted by approved local facility policies and procedures.
  • Observes and documents a patient's response to medications administered and report any change in patients' condition to RN or Md/do, as appropriate.
  • Recognizes and institutes emergency measures when indicated, promptly seek the assistance of the RN or Md/do and assist in resuscitation procedures in cardiac and/or pulmonary arrest.
  • Work Schedule: Monday – Friday, 8:00am – 4:30pm Recruitment Incentive (Sign-on Bonus): Not Authorized Pay: Competitive salary and regular salary increases.
  • When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
  • Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
  • Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
  • Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
